Argentina: Public Health in Urban Environments

Abstract

Las pruebas genéticas son una herramienta útil para la prevención y el diagnóstico de defectos congénitos en las mujeres embarazadas. Esta investigación explora el acceso y uso de estas pruebas para mujeres que residen en la Ciudad Autónoma de Buenos Aires en el año 2017. Se trata de un tema amerita una investigación porque la tecnología para hacer estudios genéticos todavía es algo novedosa. Además, a través de las pruebas genéticas es posible detectar si el feto tiene un alto riesgo para desarrollar un defecto congénito, el cual es una de las causas más importantes de la mortalidad prenatal y el aborto espontáneo. Los objetivos del presente trabajo son: indagar sobre las pruebas genéticas disponibles en Argentina, describir las instituciones en CABA que realizan pruebas genéticas y conocer las opiniones de los profesionales sobre las prácticas de las mujeres en relación a la disponibilidad y acceso a estas pruebas.

La metodología empleada fue de tipo cualitativo y se basó sobre entrevistas en profundidad con profesionales de salud. Estos profesionales fueron tres genetistas y dos obstetras y ginecólogos, para un total de cinco entrevistas. Se diseñaron entrevistas semiestructuradas, a partir de una guía con preguntas abiertas, lo cual permitió a los profesionales responder con libertad sobre sus propias experiencias profesionales con las pruebas genéticas. Además, se obtuvo información de fuentes secundarias, tales como estudios y artículos científicos.

Las entrevistas mostraron que existe baja disponibilidad de pruebas genéticas y una dificultad en el acceso a ellas, particularmente para las mujeres del sector público del sistema de salud. Un obstáculo significativo para estas mujeres es la limitación económica: muchas de ellas no están en condiciones de solventar los costos que demandan las pruebas genéticas de mayor complejidad, aunque estas pruebas podrían ser de utilidad y ayuda. En general, tanto para el sector público como para el sector privado existe una falta de información disponible para las mujeres y de igual modo a los médicos, porque la genética es un tema poco abordado en la formación universitaria. Otro factor que complica el uso de las pruebas genéticas es que el aborto es ilegal en Argentina, salvo algunas excepciones como los casos de violación, la puesta en peligro de la salud de la madre o la discapacidad mental de la madre.

Genetic tests are a useful tool for the prevention and diagnosis of congenital disorders, or in other words, birth defects, in pregnant women. This investigation explores the accessibility and use of these tests for women residing in the Autonomous City of Buenos Aires in the year 2017. This is a theme worth investigating because the technology to perform genetic tests is still novel. Furthermore, genetic testing can detect if a fetus has a high risk for developing a congenital defect, which is one of the most common causes of prenatal death and miscarriage. The objectives of the following work are to: investigate the genetic tests available in Argentina, describe the institutions in Buenos Aires that carry out genetic testing, and become familiar with the opinions of professionals about women’s practices in relation to the availability and accessibility of these tests.

The methodology was completed in a qualitative manner and was based on in-depth interviews with health professionals. These professionals included three geneticists and two obstetricians and gynecologists for a total of five interviews. The interviews were semi-structured with open ended questions, which allowed the interviewees to respond with liberty about their own professional experiences with genetic tests. Information was also obtained through secondary sources such as previous scientific studies and articles.

The interviews showed that there is low availability of genetic tests and that they are still difficult to access, particularly for women who are a part of the public sector of the health system. A significant obstacle for these women is the economic constraint. They cannot pay for the more complex genetic tests even though these tests could be useful and help them. In general, for both the public and private sector there lacks available information about genetic testing for women as well as doctors because genetics is a subject that is under addressed in higher education. Another factor that complicates the use of genetic testing is that abortion is illegal in Argentina except for cases of rape, endangering of the mother’s help or if the mother is mentally disabled.
