Argentina: Public Health in Urban Environments

Abstract

Información contextual: Desde que fue publicada la resolución de la Alma-Ata, varios sistemas de salud al nivel mundial han implementado programas de agentes sanitarios para ayudar en el cumplimiento de la estrategia de la Atención Primaria de la Salud (APS) y el aseguramiento de la salud como derecho humano. Sin embargo, en los últimos años, varios investigadores identificaron obstáculos comunes que enfrentan los agentes sanitarios en el alcance de sus tareas como resultado de la falta de apoyo que reciben por el sistema de salud. En forma sucesiva, dicha falta de apoyo ocasionó daño potencial para su capacidad de contribuir eficazmente en la estrategia de la APS. El sector público del sistema de salud de la Argentina tiene, por ley, un compromiso con la estrategia de la APS y con la salud como derecho, y, por lo tanto, algunas provincias han incorporado programas de agentes sanitarios para contribuir a esto, incluso del Ministerio de Salud de Mendoza. El objetivo central de la investigación fue indagar cómo es la experiencia de los agentes sanitarios mendocinos, en términos del apoyo que reciben por el sistema de salud provincial.

Metodología: La investigación fue cualitativa, exploratoria y no longitudinal, y se basó en dos grupos de entrevistas semi-estructuradas. En primer lugar, se hicieron dos entrevistas con representantes del Ministerio de Salud de Mendoza, para conocer las características del programa desde la perspectiva oficial. En segundo lugar, se llevaron a cabo entrevistas con cuatro agentes sanitarios de la provincia, para explorar sus percepciones sobre el apoyo que reciben por parte del sistema de salud.

Resultados: Primero, se presenta la estructura teórica del programa provincial de agentes sanitarios desde la perspectiva oficial. Esta exposición se centra en gran parte en el hecho de que, actualmente, el programa se encuentra en un momento de transición por la promulgación en 2014 de la primera ley que formaliza el papel de los agentes sanitarios en el sistema de salud de la provincia. Se identifican factores estructurales—como la crisis económica y recortes en el Ministerio de Salud Nacional—que en la práctica pueden complicar el apoyo que provee el sistema de salud provincial actualmente. Mediante las reflexiones que compartieron los agentes sanitarios, se revela que, aunque este grupo está generalmente satisfecho con el apoyo que recibe por el sistema de salud, todavía se destacan desafíos que pueden obstaculizar sus tareas y su capacidad de cumplir la estrategia de la APS.

Contextual Information: Since the publication of the Declaration of the Alma-Ata,various health systems around the world have implemented programs of community health workers to assist in the fulfillment of Primary Health Care (PHC) strategy and the assurance of health as a human right. Nevertheless, in recent years, several researchers have identified obstacles commonly faced by community health workers in the completion of their work as a result of a lack of support provided by the health system. This lack of support has the potential to damage the capability of community health workers to contribute effectively to the PHC strategy. The public sector of the Argentine health system has, by law, a commitment to the PHS strategy and health as a right, and as such, various provinces have incorporated community health worker programs to contribute, including the Ministry of Health of Mendoza. The central objective of the investigation was to explore the experience of the community health workers of Mendoza province, in terms of the support that they receive from the provincial health system.

Methodology: The investigation was qualitative, exploratory, and non-longitudinal, and based in two groups of semi-structured interviews. Firstly, interviews with two representatives of the Ministry of Health of Mendoza were conducted in order to characterize the community health worker program from the official/provincial perspective. Secondly, four community health workers of the province were interviewed in order to explore their perceptions of the support that they receive from the health system.

Results: Firstly, the theoretical structure of the provincial community health worker program from the official perspective is presented; this discussion centers largely around the fact that the structure is currently in a moment of evolution due to the 2014 passage of the first law to formalize the role of the community health workers in the health system of Mendoza. Structural factors such as the economic crisis and cutbacks by the National Ministry of Health are identified as factors that currently complicate the support that the provincial health system is able to provide to the program.

The reflections shared by the community health workers reveal that, although this group is generally satisfied with the support that they receive from the health system, notable obstacles remain, and could limit their ability to contribute to the fulfillment of the PHC strategy in Mendoza.
